Description
El proceso de toma de decisiones en la administración pública es abordado desde diferentes ópticas. Se ha enfatizado en el diseño de políticas basadas en evidencias, como la evaluación de tecnologías sanitarias, pero estas no son el único sustento sobre el cual se diseñan. En pandemia se observó una gran variabilidad en las tecnologías sanitarias recomendadas y cubiertas por distintos ministerios de salud, en un país federal como Argentina. Se indagó acerca de procesos y resultados de la toma de decisiones sobre tecnologías sanitarias en los ministerios de salud de Argentina en el contexto de pandemia. Se realizó una triangulación de métodos cuali-cuantitativos, recolectando información del
período 2020-2021 mediante revisión documental, encuestas y entrevistas en profundidad a miembros de los ministerios de salud de la Nación y provincias argentinas. Se identificó una importante variabilidad en las tecnologías recomendadas y cubiertas por los distintos ministerios de salud. Ninguno adhirió completamente a las políticas recomendadas por la OMS. Influyeron distintos criterios técnico-políticos, conformación de equipos asesores, relaciones de poder dentro y fuera de la administración, presión
social, de los medios, académicos, poderes judicial y legislativo. La toma de decisiones
en la administración pública en pandemia mostró ser desordenada, compleja y no lineal.
The decision-making process in public administration is approached from different perspectives. Evidence-based policy design, such as health technology assessment, has been emphasized, but these are not the only underpinnings on which they are designed. During the pandemic, great variability was observed in the health technologies recommended and reimbursed for the different ministers of health, in a federal country like Argentina. We inquired about the processes and results of decision-making on health technologies in the ministries of health of Argentina in the context of the pandemic. A triangulation of qualitative-quantitative methods has been carried out, collecting information from the 2020-2021 period through documentary review, surveys and in depth interviews with the national health ministers and Argentine provinces. An important variability in the technologies recommended and covered by the different Ministries of Health was identified. None ministry fully adhered to WHO recommended policies. Influence of different technical-political criteria, conformation of advisory teams, power relations inside and outside the administration, social pressure, media, academics, Judicial and Legislative powers were identified. Decision-making in the public administration during the pandemic proved to be disorderly, complex, and non linear.
