En esta tesis, describimos los condicionantes que influyeron a la hora de institucionalizar la participación ciudadana en salud (PCS) en un programa de salud implementado en el período 2018/2019 en el Área Metropolitana de Buenos Aires (la Red AMBA). La PCS en este trabajo es considerada como un proceso de adquisición de poder ciudadano para proteger, cuidar y remediar los medios de vida y desplazar la violencia hacia la misma. Las asimetrías en el poder, en el conocimiento y en los determinantes sociales en un mundo cada vez más globalizado y con mayor desigualdad social, influyen en que, la PCS se observa pobremente implementada cuando se analizan las prácticas concretas de los programas de salud. Mediante una metodología de escenas, describimos la tensión de fuerzas operando en el sistema, y analizamos cómo las mismas configurarían los tres niveles de la forma del sistema relacional donde ocurriría la PCS (acción, contexto y metacontexto) según el enfoque de sistemas complejos. En el flujo histórico-político, analizado desde el retorno de la democracia en Argentina, a través del relato de los propios funcionarios que implementaron programas previos, la resultante de la tensión de fuerzas habría condicionado la construcción de la dimensión estructural sistémica para que tenga lugar la PCS en la Red AMBA. La expresión de fuerzas de igualdad de derechos, en los actores de las escenas, condicionó la construcción de la dimensión relacional del sistema. Las categorías que se asociaron a estas dimensiones se nutren de los atributos de un poder horizontal, de acciones disruptivas de los funcionarios y de conflicto con el marco instituido en los lugares donde se implementó la Red AMBA. En las interacciones donde la participación del sujeto se observa limitada, sea por preservación frente a la violencia del poder vertical o por conductas cínicas renegadas del ideal del rol, el sistema es reproducido. La Red AMBA presenta indicios de tecnologías sociales transicionales para la implementación de programas. Los procesos participativos serían generadores de prácticas socio-médicas desplazando la reproducción de la violencia de las interacciones sociales, dando nuevas formas institucionales.We contribute with some conditioning factors that may influence the institutionalization of community involvement in health (PCS) that were analyzed through a case study in a health program, known as AMBA Network. This program was implemented in the 2018-2019 period, in the Buenos Aires Metropolitan Area. PCS is considered a process of acquisition of citizen power to protect, take care of and remedy livelihoods displacing social violence. (Oakley & Health Organization, 1990); (Fassin, 2004). Power and knowledge asymmetry, as well as imbalance in social determinants in an increasingly globalized world and with rising social inequality pose a challenge for a real state governance that provides equality of rights. (Herrero et al., n.d.). Even when PCS is enunciated in most sanitary policies, its poor implementation can be observed when it comes to the analysis of concrete practices of health programmes (Menéndez, 1998). The tension of forces operating in the system (Samaja, 2004b) is described through scene methodology (Goffman et al., 1981) and the way in which they may constitute the three levels of the relational system where PCS would occur (action, context and metacontext) (Keeney & Wolfson, 1987). Within the framework of the historical-political flow analyzed from the return to democracy in Argentina, the outcome of the tension of forces seems to have conditioned the construction of a systemic structural dimension for PCS to take place in AMBA, according to the very public officials that implemented previous programmes. The expression of equal rights forces in the actors representing the training scenes that were carried out by the programme appears to influence the conditioning of the construction of the relational dimension of the system. The categories associated with these dimensions are nourished by the properties and attributes of a horizontal power, by disruptive actions on the part of public officials and by conflict within the instituted framework in the places where the AMBA Network was implemented. It is concluded that the interactions where the involvement of individuals finds itself constrained by preservation in the face of vertical power violence or by cynical behaviour, far from the role-ideal, seemingly do not express equality forces since dialogue between the skills and knowledge of the actors is not enabled and, therefore, the system does not get to achieve a dynamic, adaptive state. This dissertation intends to contribute that the reproduction of gregarious, archaic and/or global forces in the actions and behaviour of public officials may violate the system by preventing it from reaching its potential and condition the PCS institutionalization process negatively in terms of the expression of forces of rights.
